Shine - Spina Bifida Health Survey

We know your health is important to you and it’s important to us too. That is why we are setting out to find out more about the health issues that you face and your experiences of using health services.

We’d like to get a proper picture of what it’s like out there and how happy you are with your health and the help and support you get.

We want this information so that we can work as effectively as possible to promote and support the health of our members.

We are starting with a short survey for members with spina bifida which you’ll find below. Please, if you can, answer the questions in this survey and send it back to us.

Just to make it a bit more attractive, anyone completing the survey will be entered into a prize draw with £100 first prize and £50 second prize!
Later, we will have a survey for members who have hydrocephalus only.

So, if you or your child, has spina bifida, we’d be really grateful if you could fill in as much of this survey as possible and send it back to us.

What you tell us will help us begin to build a picture of the overall situation for people with spina bifida in England, Wales and Northern Ireland.
